1. Complete and sign all forms, keep extra copies for your records

2. Make copy of driver’s license or state issued identification along with copy of most recent statement from
    your current IRA or 401(k) account

3. Scan all of the above and email to preciousmetals@theentrustgroup.com

4. Mail original hard copy along with driver’s license copy and IRA or 401(k) account statement to the
    address listed above
